If you have moved into our practice area you are welcome to register with us and our reception / signpost team will be happy to guide you through the procedure. You will need to complete a registration form and a health questionnaire, which can be downloaded from the website or with prior arrangement can be collected from reception. You will need to form of (ID) identification  less than 3 months old with your name and address, 1 of which should show a photo of you. We can only accept copies. Please do not bring or leave Originals.

Even if you are fit and healthy, the doctors or Pharmacist would like to speak with soon after for a consultation, to take details of your medical history and current medication, if applicable. This consultation is very useful to the clinician, because it can take several weeks to receive your previous medical records, due to the complex application procedure.

Please ask for a new patient consultation appointment four weeks or so after registration   Please have details to hand of any medication that you are taking regularly.

If you are or have a carer, we would be grateful if you could complete a form available from online or we can post it to you.

If You Change Your Name or Address

Please inform us of any changes. It would also be useful to have your home and mobile telephone numbers. If you are ex-directory and are concerned about divulging your telephone number, we can assure you that this is required for surgery (and possibly hospital) purposes only. Change of address forms are available from reception.
